Thank you for sharing... I know your team has been through a lot!
What brand stops are you using? Are they aluminum or titanium? Any other insight into a possible cause?

We use titanium stops and they are also threaded on one side eliminating the need for a nut. Another thing we do is to use never sieze on the threads and the washer to help make sure the torque is on the bolt and not on the washer "drag".

After coming from 11th to 2nd in first feature, starts 15th and while running 2nd late and running down leader, loses right front stop, (according to him in post accident interview), and clobbers turn 2 wall. Emerged un-injured. Justin Henderson won 1st feature, Mark Dobmeir wins the second. Given this is the same issue that ended Jason Leffler's life, what needs to be done to address this issue?

We believe we that the bars are sliding out with the arm attached and jamming in the combo arms(Steering).
We actually attached a drag link tether from the arm to the frame(lower part). Nothing is in the way so you can serice your bars and will NOT interfere withe the action of the arms.......(...a Coonass solution) cost of about $20..

A friend of mine had a problem with the pinch bolt in the stop. It was a few threads short and was tightening against the shoulder of the bolt before the stop pinched the bar tight enough. They destroyed two cars before figuring out the problem. It seems like a simple thing but can be overlooked. Everyone please take a look at your pinch bolts, on both ends, and make sure you have plenty of threads.

FYI, I just weighed one of my torsion bar rods. It is all standard weight 3/8" steel, and it weighs 14.2 oz. That is a full length piece of 3/8" rod, 2 fender washers, 2 short pieces of 3/8" hose to keep it centered, a regular nut TIG welded to one end, and a full thickness nylock nut on the other end. Yes, there may be a better way. But if you don't already have that better way, maybe for $5 and less than a pound, you ought to be putting one of these on. If you are an adult and you maintain your own car and don't want to, well that's on you. But if you are a crewchief, please don't send your driver out without something in place to at least try to prevent this from happening again.
